Modafinil was originally developed for narcolepsy and later approved for shift work sleep disorder and obstructive sleep apnea related sleepiness. Those indications remain the strongest. Over the last decade, off label interest has shifted toward cognitive enhancement in healthy adults, with studies looking at memory, planning, motivation and creativity.
The results are consistent in one respect: modafinil improves wakefulness and executive function in sleep deprived people more than in well rested people. In rested subjects, the effects are smaller and more variable. Some studies show benefits on complex tasks requiring sustained attention; others find no improvement or even slight impairment on creative divergent thinking.
Newer research is exploring modafinil in depression, ADHD and neurodegenerative models. Early findings are promising but preliminary. The compound is unlikely to be a universal smart drug, but it may have niche applications where sleep disruption or fatigue is the primary limiting factor.
